Diabetes Educator
General summary/purpose: The Diabetes Educator is a professional nurse, registered dietitian, clinical pharmacist or social worker. This role is an integral part of the JHOC diabetes center inter-professional team, providing comprehensive and patient centered diabetes self-management training for people with diabetes. The Diabetes Educator works with the DSMT program manager, diabetes center staff and community partners to innovate, plan, and implement diabetes care and education programs. The ideal candidate will be a self-starter with strong interpersonal skills and be committed to providing an outstanding patient experience.
