The Hemophilia Center at OHSU is the only federally-recognized Hemophilia Treatment Center (HTC) in Oregon and southwest Washington. The HTC provides integrated care to children and adults with hereditary bleeding and clotting disorders. OHSU is one of only eight regional core hemophilia centers in the U.S., leading the Mountain States Regional Hemophilia Network. The Hemophilia Center serves people with inherited blood disorders, such as von Willebrands disease, hemophilia a, hemophilia b, other factor disorders and platelet disorders, as well as those affected by inherited clotting disorders.
Reporting to the Specialty Pharmacy Operations Manager, this position is responsible for the provision of specialized clinical and/or MTM services to adult and pediatric patients who are treated for hemophilia and other bleeding disorders in a community/ambulatory/outpatient setting.
This position is responsible for:
Direct clinical supervision of patients, the supervision of support staff and ensuring compliance with all legal, regulatory and institutional directives.
The pharmacist will interact directly with patients, prescribers and care coordinators in the Hemophilia Center at OHSU to develop an interdisciplinary approach to patient care.
Additionally, the pharmacist will be responsible for the development and implementation of additional Hemophilia Pharmacy service opportunities and will be expected to contribute to the financial viability of the program, department and institution.
BS in Pharmacy or Pharm.D.
Community pharmacy practice experience or inpatient pharmacy practice experience.
State of Oregon Board of Pharmacy Pharmacist license.
Pharmaceutical care principles. Disease related modification of drug therapy.
Project management skills and experience in contemporary medication use systems and electronic medical record systems.
Strong interpersonal communication skills.
Systematic problem solving skills.
Ability to work independently with minimal supervision.
Competency in patient interviewing and teaching skills.
Excellent knowledge of third party payors, insurance billing, and prior authorization guidelines of Specialty Medication.
Working knowledge and skill usingMS-Word, Outlook, Excel, Powerpoint.
Advanced Pharmacy Practice Residency, BCPS- OR- 5 years of equivalent experience.
Experience in Specialty pharmacy practice, including hemophilia/bleeding disorders either inpatient or outpatient.
Inpatient and / or outpatient hospital pharmacy practice.
Three years of health system experience.
Experience in managing hemophilia patients in an ambulatory setting.
Experience with factor product prescriptions, labeling and documentation in CPR+ and Epic.
Specialist Certifications (BCPS, BCOPS, etc)
State of Oregon Pharmacy Preceptor licensure.
