Social Worker 2, Lombardi Comprehensive Cancer Center - Georgetown University Medical Center
The Social Worker 2 provides advocacy and other support services to patients with hemophilia and related bleeding disorders and their families as relates to hospitalization issues, dealing with illness and post-hospital planning and disease complications including HIV. They assist patients and the Administrator in managing patients involved in the 340B program, and coordinate with the Center for Bleeding Disorders (CBD) for the federally funded comprehensive Hemophilia Treatment Center (HTC) at Georgetown University Medical Center.
Reporting to the Administrator, and receiving guidance from the Medical Director and the HTC Physicians and Nurse Coordinator, the Social Worker 2 has duties that include but are not limited to:
Social Work Services
Provides social work services to identified hemophilia and bleeding disorder patients and families regarding the impact of illness and hospitalization.
Serves as a coordinating link among patient, family, hospital and community resources to assure continuity of care in the hospital and in post-hospitalization.
Coordinates insurance and reimbursement issues, including patient involvement in the Center's 340B program.
Collaborates with other health care professionals.
Facilitates communication regarding relevant social issues of patients and families to health care teams through recording in medical charts, referrals and participating in meetings.
Creates, distributes and collects yearly needs assessments from active patient database.
Represents social work perspective and serves as HTC/CBD representative to designated hospital committees and community organizations.
Works with the local Hemophilia chapter as our Center's representative and actively participates with their fundraising and educational programs.
Actively participates in grant-writing initiatives.
Actively develops publication material for patient and community education, including a quarterly newsletter.
Organizes and presides over patient support groups.
Serves as Center liaison to the Consumer Advisory Board and coordinates activities as needed.
Identifies training needs, locates resources, and promotes their own professional development.
Master's degree in Social Work
Experience with adult and pediatric patients
Experience with complicated diseases and HIV
