The Department of Medicine, Division of General Internal Medicine seeks a Patient and Community Outreach Specialist (PCOS) to be an integral and valuable member of the SiNERGe northeast sickle cell network. SiNERGe is a grant-funded collaborative supported by the Health Resources and Services Administration (HRSA). The northeast regional sickle cell collaborative is coordinated out of the Johns Hopkins University and covers 13 Northeast states, Puerto Rico, the US Virgin Islands, and the District of Columbia. The primary goal of the program is to help improve health care services and outcomes for children, adolescents and adults in the northeast region of the U.S. who have sickle cell disease. Project activities include, but are not limited to, provider and patient education, patient engagement, data collection, and quality improvement initiatives.
The PCOS needs to embody the fundamental objective of this position; to seamlessly work with others to identify and promote SiNERGE Network activities and promote the health and well-being of individuals with sickle cell disease. Effectiveness in this position is developed and maintained by the iterative, proactive, interactive and collaborative nature of the position.
The ideal candidate will be responsible for generally supporting the program’s activities by connecting and engaging local patients with sickle cell to resources and care, engaging regional/national community via social media and other outreach activities, serving as a liaison with regional community-based organizations in the SiNERGe network, and assisting in the operations of SiNERGe educational activities and events.
The PCOS will engage the sickle cell community across the SiNERGe Network and individual patients alike. Individual patient engagement will include administering assessments to identify potential home and community/socially oriented healthcare access and treatment adherence barriers. Upon confirmation of preliminary findings, the PCOS will assist in the development of a plan in collaboration with healthcare team, to minimize or eliminate identified barriers to care and adherence. This individual will assist patients directly in navigating the care system and linking patients to needed resources.
The PCOS will be able to provide basic health education, lifestyle counseling/health coaching to individuals with sickle cell disease.It is also expected that the PCOS will maintain a highly visible presence in the community and across the SiNERGe network and will actively promote and assist in operationalizing activities of the SiNERGe network.
All duties and responsibilities will require data collection and reporting. PCOS will be responsible for working with the project director(s) and project manager/coordinator(s) to evaluate patient and community outreach strategies and priorities.
Specific Duties and responsibilities:
The position involves a wide range of activities related to outreach, education, and engagement of patients and the community, on behalf of SiNERGe and the Johns Hopkins Sickle Cell Center for Adults.
Initiate, build and maintain relationships with patients in need from the clinic and community sites- offering education, support, and guidance in medical and non-medical supportive services that are available in our program and in the community.
Assist clients in navigating a complex medical system by building trust and a professional working relationship including coordinating and accompanying participants to medical, mental health, and social service appointments as needed.
Assess the patient’s strengths as well as needs to address individual and community barriers to care and intervene as appropriate.
Provide support, encouragement, education, and resource materials to patients and their families.
Provide direct service to potential patients who have fallen out of care. This may include going into the community or to a patient home to locate out of care patients to re-engage them into care following established policies for outreach activities.
Document services and support provided in the electronic medical record system.
Telephone contact with patients and community-based agencies.
Liaison with community-based organizations in the SiNERGe network and facilitate community partnerships
Assist with activities to maintain and expand the involvement of community partners
Assist in collaboration or production of a recurring sickle cell support group
Assist with the planning and implementation of new and existing outreach and education programs including: educational events, social media pages, webpage, newsletter, print materials, and other broadcasts that will increases visibility, increases patronage, and improves the SiNERGE network
Effectively communicate the SiNERGe network’s mission, programs, and initiatives to a broad range of constituencies
Attend and assist with events that provide general visibility for SiNERGe, including virtual education events, community conferences, and health fairs
Track and report information on activities and reach, and other general record keeping
Attend and participate in regularly scheduled staff meetings and conference calls
Other duties as assigned in support of the SiNERGe collaborative or Johns Hopkins Adult Sickle Cell Center goals.
These responsibilities would be conducted remotely until there is a return to normal JHU and Medicine operations post-national pandemic emergency. At that time these responsibilities may involve travel in the Greater Baltimore area, including travel between several Johns Hopkins medical campuses, patient homes, and to community events. A flexible daily schedule will sometimes be necessary, based on patient needs and the time and location of community outreach/education events.
These responsibilities will be carried out under the supervision of the SiNERGe manager/coordinator, but also requires close collaboration with the directors, faculty, and staff of the SiNERGe coordinating center team and the Johns Hopkins Adult Sickle Cell Center.

High school diploma/GED required. A minimum of two years’ experience (in health-related, community outreach, or working with complex-needs patient population.
Bachelor's degree or certificate in related discipline is preferred. Additional education may substitute for required experience, to the extend permitted by the JHU equivalency formula.
Formal training in counseling preferable
Experience working with people with unstable housing, complex medical issues, cognitive impairments is preferred.
Experience working with individuals with sickle cell disease strongly preferred.
Knowledge of some or all of the following: strengths-based case management, motivational interviewing, and principles of harm reduction preferred.

Intermediate to high profiency/comfort using technology including experience with:

Sitting, standing and walking for extended period. Reaching by extending hand(s) or arm(s) in any direction. Finger dexterity required to manipulate objects with fingers rather than with whole hand(s) or arm(s). Ability to move standard equipment through a hospital or clinical environment.
