Social Worker II - Georgetown University Medical Center

Requirements

The Social Worker II provides advocacy and other support services to patients with hemophilia and related bleeding disorders and their families as relates to hospitalization issues, dealing with illness and post-hospital planning and disease complications including HIV. S/he assists patients and the Administrator in managing patient involved in the 340B program. The incumbent also provides social work support, leadership and teaching functions, needs assessments and coordinates the Consumer Advisory Board for the federally funded Comprehensive Hemophilia Treatment Center at Georgetown University Medical Center. Acting as the only social work position in the Hematology research group and reporting directly to the Administrator of the research program, the Social Worker II has additional duties that include but are not limited to:
  • Provides clinical social work services to identified hemophilia patients and families including counseling regarding the impact of illness and hospitalization.
  • Functions as a coordinating link between patient, family, hospital and community resources to assure continuity of care in the hospital and post hospitalization.
  • Functions to coordinate insurance and reimbursement issues including patient involvement in the center's 340B program.
  • Collaborates with other health care professionals and facilitates communication regarding relevant social issues of patients and families to health care teams through recording in medical charts, referrals and participating in meetings.
  • Creates, distributes and collects yearly needs assessments from active patient database.
  • Represents social work perspective and serves as HTC representative to designated Hospital committees and community organizations.
  • Works with the local Hemophilia chapter as a representative of our center and actively participates with their fundraising and educational programs.
  • Actively participates in grant-writing initiatives and develops publication material for patient and community education including a quarterly newsletter.
  • Coordinates activities and is the center liaison to the Consumer Advisory Board.
  • Assumes responsibilities for identifying training needs and locating resources and promoting their own professional development.


Requirements and Qualifications
  • Masters in Social Work
  • Experience with adult patients
  • Experience with complicated diseases and HIV is desirable
